A medical practitioner needs to verify your impaired mobility and state that you meet the eligibility requirements. Particulars that differ from the general eligibility guidelines include:

- You are permanently blind or visually impaired

What is the Indiana application process for Disability Parking Plates and Placards?
You can get the Application for Disability Plate or Parking Placard (State Form 42070) by going to your nearest Indiana Bureau of Motor Vehicles Office or printing the document online.
If you’re applying for a temporary disability placard, you will need to mail in a $5 fee with your application. Disability license plates include registration fees and taxes; learn more about specific fees on the Plate and Registration Fee Chart.
You and your qualified physician just need to complete and sign the application with all necessary information. Once finished, just mail or bring in the application to:

How long are Disability Parking Plates and Placards Valid in Indiana?
Depending on the handicap parking plate or placard, temporary or permanent, the validity varies.
- Permanent Parking Plate: valid until expiration date on license plate
- Permanent Parking Placard: valid until date specified by your doctor; if it has expiration date, it will expire then
- Temporary Parking Placard: valid up to 6 months
Disability License Plates need to be renewed before the expiration date listed on the plate or placard. Disability Plates can be renewed online at myBMV.com or by going to a Indiana BMV Office.
Disability placards issued to an individual with a permanent disability, can be renewed at a BMV Office. Your subsequent disability placard won’t have an expiration date.
Are there Indiana Disabled Veteran Plates?
Disabled Hoosier Veteran license plates are available to any Indiana resident veteran with a service-connected disability. Your disability needs to be at least 50% service-connected, and of that 50%, 30% must be related to a mobility impairment.
You will need to provide a letter verifying your service-connected impairment from the Department of Veterans Affairs. In addition, you will need to present a copy of your military discharge.
Once you receive the eligibility verification, you will need to fill out the Certification of Eligibility for Veteran License Plates. When you’re done, you mail or fax it to:

Can I report Handicap Parking Abuse?
Unfortunately, there are instances where handicap parking spaces, plates, and placards are taken advantage of by those who don’t need have impairments.
If you happen to see someone swiftly moving from an empty car in a handicap space, there’s a chance they shouldn’t park there.
Anyone who parks in a spot dedicated for disabled individuals, without the proper plate or placard could receive a fine. By doing this, you would be committing a Class C misdemeanor under Indiana law.
You can report this behavior by contacting local authorities or property owners of the space. Be sure to provide details of the incident and license plate information.
