Are you eligible for Handicap Parking Placards or Plates in Arizona?

A certified health professional needs to certify your impaired mobility with the eligibility requirements. Specifics that vary from the general eligibility guidelines on the home page include:

  • You can’t walk 200 feet without rest
  • You’re restricted by lung disease, specifically with a forced expiratory volume (FEV) for 1 second, when measured by spirometry, is less than 1 liter or your arterial oxygen tension is less than 60mm/hg at rest
  • You’re severely limited to walk because of an arthritic, neurological or orthopedic condition.

Organizations that transport disabled individuals, such as nonprofits, can also be eligible to receive disability parking placards or plates.

What is the Arizona application process for Handicap Parking Placards and Plates?

In order to apply, you need to fill out the Disability – Hearing Impaired Plate/Placards Application (Form 90-0104). You can access the application by going to your local Arizona Motor Vehicle Division (MVD) Office or printing the document online.

Legal guardians of a disabled person under 18 years old need to sign and provide your relationship to the applicant to receive handicap plates or placards.

If you’re hearing impaired you will need to have an audiologist certified by the American Speech, Language and Hearing Association complete your initial application. You will then receive a special Hearing Impaired Plate to alert law enforcement and emergency vehicles of your condition.

You and doctor just need to complete and sign the application with all necessary information. Once you finish the application, you can either bring it in to your nearest MVD Office or mail it to:

If you would like to possess a special or personalized plate, you will need to complete the Special Plate Application (Form 96-0143). Any of the special plate options need to be approved by the specified organization.

The Motor Vehicle Division will process your application and if approved, they will mail the plates or plates to your home address.

How long are Handicap Parking Placards and Plates Valid in Arizona?

Depending on the handicap parking permit, temporary or permanent, the time range and policies vary.

  • Temporary Placards:
    Valid for up to 6 months
    If condition continues after 6 months, you must reapply
  • Permanent Placards and Plates:
    Valid for 5 years
    May be renewed

Permanent placards and plates need to be renewed by the expiration date and may be made by completing the Placard Renewal/Replacement Request (Form 40-0112). After finishing the request, either bring it in or mail it to your nearest MVD Office or the Phoenix address listed above. Renewals require a $5 fee.

Are there Arizona Disabled Veteran Plates?

Disabled Veteran License Plates are available for any Arizona resident veteran. Your disabling condition needs to be service-connected as certified by the Department of Veterans Affairs or military service that discharged you.

You will need to complete both the Disability – Hearing Impaired Plate/Placard Application and the Special Plate Application to apply for disabled veteran plates. You may also order the specialty plate online at the Plate Selection portion of the Arizona Department of Transportation website.

Once you receive certification and you complete the applications, you can mail it to:

There’s a standard fee of $25 and a personalized fee $50 on the veteran plate you select.

After experiencing the benefits of the disabled veteran plate, you just need to renew the plate when specified. Also, you can appreciate the benefits of Social Security Disability after filing a disability claim.

Can I report Handicap Parking Abuse?

There are instances, unfortunately, where handicap parking spaces, permits, and placards are taken advantage of by those who don’t have a disability.

If you happen to see someone easily moving from an empty vehicle in a Handicap space, there’s a possibility they shouldn’t park there.

Notify authorities and/or property owners as soon as you observe a driver abusing a handicapped parking space.

Misusing handicap plates and placards are in violation of Arizona law. Fines and penalties could be charged to the owner or operator of the vehicle. The handicap plate or placard may also be recalled if abused.

Arizona MVD Field Offices are distributed throughout the state and they offer a variety of services, including vehicle and disability permits, registration and title transactions, and license plate renewals, among other things.
