Multiple debilitating conditions

I have type 2 diabetes, neuropathy in my feet, heel pain due to bone spur, chronic back pain, stenosing tenosynovitis (Trigger Finger) in 3 different fingers, degenerative arthritis in my left shoulder and right elbow, chronic fatigue, bloating, and excessive weight gain (40 lbs since I went on insulin 18 months ago).

I work full time in a physically demanding job and it is affecting my job performance and I don't know how much longer I can hold up. I can't go without income and the disability process seems to expect me to do just that.

I feel like I'm at my breaking point but I keep going because I need my job or more accurately need the income. What should I do? I can't keep this up much longer.
