degenerative disc disease

I have degenerative disc disease,C7 radiculopathy, severe carpultunnle in both hands,ADHD, and mild to moderate rheumatoid arthitus in my knees. Would I qualify if so is there a deadline on how long you have to file,how do I file and what would I need to give them to help me to get my benefits
